Frank Ejzak passed away peacefully, with family at his side, on October 30, 2006. Frank was born on October 3, 1916 in Czarnia, Poland and was the son of Jan and Rozalia Ejzak. He married Josephine Toczycka on December 2, 1950 at St. Fidelis Church (Chicago) and resided in Chicago, IL. Frank served the Polish army during World War II, lived in Germany after the war and immigrated into the United States in 1950. He was an expert tool-and-die maker and retired in 1981. Since then he's been an active member of the St. Hyacinth's Basilica Choir and the Polish Veterans.

Frank is survived by his loving wife of almost 56 years, Josephine; son Edward and his wife Paula; son Richard, his wife Karen and grandchildren Larissa and Garrett; son Andrew, his wife Susan and grandchildren Nicole and Sean; brother Lucjan. He also leaves behind many nieces, nephews, cousins, relatives and friends.

The family wishes to express a sincere appreciation for all of the prayers and condolences that they've received during this difficult time. Special gratitude goes to Emily Lipowicz and Lottie Toczycki (aunts to Frank's children, and sisters of Frank's wife Josephine) for their help in making final arrangements.

Pogrzeb odbedzie sie w sobote, dnia 4 listopada o godzinie 9:00 rano z zakladu pogrzebowego pnr. 4540 W. Diversey Ave. W Chicago IL, do bazyliki sw. Jacka (msza sw. o godz. 10:00 rano) a stamtad na cmentarz Maryhill na dzialke kombatancka.

O czym zawiadomiaja, pograzeni w glebokim smutku: Jozefa ( z domu Toczycka), zona; Edward (Paula), Richard (Karen) i Andrew (Susan) synowie; Nicole, Larissa, Garrett i Sean, wnuczki i wnukowie; Lucjan, brat oraz pozostala rodzina w Polsce i Ameryce.
